Estrogen cream may work, although given what you've told us about the perineal repair you might like to see a urogynaecologist for examination and advice.

Insomnia is a symptom of the menopause but the treatment is estrogen replacement and not progesterone, although the drug tibolone sometimes works.

The HPV virus that cause abnormal smears is spread by sexual contact, not just penetrative intercourse, so the situation should be reassessed with that in mind.

This may be normal if you delivered recently. However, f the bleeding is increasing, offensive or you have other symptoms like pain or temperature you should be seen at the maternity unit urgently.

This may be a condition called lichen sclerosis so I would ask your GP for a referral to a gynaecologist

Yes this may be PCOS so I would advise blood tests as well and referral
