Failure of conservative measures such as you describe should prompt referral to a urologist or urogynaecologist for further investigation. You may need to have a urodynamic study to define the cause of your incontinence. I would not recommend empirically trying any further treatments at this point.

Unfortunately the menopause sometimes make things worse. Have you tried vaginal oestrogen cream - this can be beneficial in such cases.

The first step (if you haven't done it already) would be to complete a bladder diary - some patients make more urine at night than during the day - a condition known as nocturnal polyuria - this would be the first thing I would want to rule out.

This is indeed an uncommon problem. I have had a couple of patients that have responded to resection of the adenoma (I assume its in the bladder). May be worth discussing the relative merits of this with your urologist.

Sometimes making the urine less acid is good for this - you can buy over the counter preparations for this. Perhaps addressing the stress incontinence with physiotherapy or surgery if physio already tried should be considered.

This sounds like stress urinary incontinence - if home exercises have not worked I would recommend referral for specialist supervised pelvic floor physiotherapy. If this is not successful you may need to see a urologist or urogynaecologist to discuss surgery.

Have you seen a urologist - I would recommend you do. Sounds like you require some assessment of bladder function for these troublesome symptoms. The bladder can leak for many reasons and I would recommend further investigation to try and elucidate this in your case.

This can indeed be the case - in such cases I usually refer to a urogynaecologist for an opinion - sometimes find treating the fibroids helps with the bladder symptoms.

Sometimes adding in an anticholinergic tablet (Oxybutynin, Tolterodine, Solifenacin etc) to the Mirabegron can be useful. Failing that you could talk to your urologist about Botox?

You could try an anticholinergic drug (Oxybutynin, Solifenacin, Tolterodine etc) or Mirabegron which works in a slightly different way. You may even try a combination of anticholinergic and Mirabegron - could be worth discussing with your GP.
