If you have otherwise normal skin then acidic urine will not cause damage. If you have LS which is not fully treated then yes, the more acidic urine over night may cause some irritation. My preferred regimen for maintenance treatment is twice daily emollients first and foremost. They keep the acidity and water level of the skin in the normal range. I'm sorry you're having this issue. A persistent and unilateral fissure that won't heal or recurs quickly does need to be seen by a dermatologist. A further biopsy could be considered if topical therapy of the right kind remains ineffective.

I think establishing a correct diagnosis is important. If your GP can be confident enough and willing to review you to see how the area responds to treatment then that may be enough. However, if you can't get that type of follow up then yes, I think it's reasonable to ask for a dermatology opinion. ...
