Re: urination
If you have otherwise normal skin then acidic urine will not cause damage. If you have LS which is not fully treated then yes, the more acidic urine over night may cause some irritation. This can be helped by ensuring you use a good barrier type emollient ointment at night time. Remember to re-apply...

Re: labia becoming none existent
I'm sorry to hear about this problem. It does sound as if you do need a careful thorough physical examination after an adequate history has been taken from you about length of symptoms and what treatments you've used etc. Labial size does very hugely between every woman, but advice on exactly where ...

Re: Efficacy of biopsies in diagnosing Lichen Sclerosus
When I first stared doing vulval clinics (last century) we thought it was a good idea to biopsy every patient. I know that was a long time ago and along with other colleagues we've come to realise that a biopsy is not necessary if a sound clinical diagnosis can be made. I now reserve biopsies for pa...

Re: LS spreading
LS can spread to other areas such as around the anus and perineum. Spreading to the inner thighs is less common but can happen. Using super potent topical steroids in the inner thighs can be associated with skin thinning - if used for any length of time. I would suggest getting your GP to examine yo...

Re: Vulval Lichen Sclerosis and vaginal/vulval atrophy
I'm sorry you're struggling with this symptom. I wonder if it is a mixture of physical dryness due to under treated atrophy or whether you also have irritable nerve endings which are giving you the sensation of dryness eg a chaffing, sand paper like or burning sensation. If so this may be helped wit...

Re: Alternative steroid creams
I'm sorry you're having a potential reaction to topical therapy. I would speak to your GP to describe what symptoms you're having. Alternative steroid preparations can be considered but do discuss this with your GP first. They can ask your local dermatologist for advice and guidance on this if neede...

Re: Topical steroid pulse therapy
My preferred regimen for maintenance treatment is twice daily emollients first and foremost. They keep the acidity and water level of the skin in the normal range. Topical steroids are then used twice a week eg Monday and Thursday to maintain remission. They work by gently reminding the lymphocytes ...

Re: Permanent Fissure
I'm sorry you're having this issue. A persistent and unilateral fissure that won't heal or recurs quickly does need to be seen by a dermatologist. A further biopsy could be considered if topical therapy of the right kind remains ineffective.

Re: undetermined diagnosis
I think establishing a correct diagnosis is important. If your GP can be confident enough and willing to review you to see how the area responds to treatment then that may be enough. However, if you can't get that type of follow up then yes, I think it's reasonable to ask for a dermatology opinion. ...

Re: severe itch
I'm so sorry you're struggling with such a severe itch. My thoughts are as follows (not in order of importance): 1. Get your HbA1C level (measure of long-term glucose control) into the normal range with your diabetic medication. Ask your GP to check if your iron levels are in the normal range too. 2...
