A vault smear is not necessary after a hysterectomy for benign pathology such as fibroids, and won't help to determine the cause of your pain at all.

Often your cycle changes as you get older. As long as there is no bleeding between perioda or after intercourse I wouldn't worry. The pain may be due to ovulation and the way to deal with that is to suppress it hormonally i.e. the pill. The urine issue is unrelated.

If you are entitled to NHS care then this test could be ordered via a referral to a recurrent miscarriage clinic.
Its unlikely that insurers will pay for it in the UK. It would cost a few hundred pounds.

Its not only the size its the location. while it would be a difficult laparoscopy, the position my make it even more difficult and the operation would take much longer and therefore increase risks of other non-surgical complications.
