I was diagnosed with vulval Lichen Planus a year ago, after three years of symptoms. The dermatologist discharged me in May as all the inflammation had cleared after a regime of daily use of Dermovate for a month, then twice a week - and repeat. I use an emollient daily. After three weeks solely using the emollient the LP flared up again. I have gone back to the Dermovate regime, but, as soon as I think the inflammation has cleared and stop using it for a few days, it comes back in one specific area. Would I be better using the Dermovate twice a week to keep the inflammation under control? Or should I ask to be referred back to the dermatologist?

Thank you for your query. If the lichen planus recurs soon after stopping Dermovate it may help to taper the treatment more slowly such as twice daily for the first month, then once daily for second month, alternate days for third month and twice a week for the 4th month before stopping. If despite this the condition still flares then I would suggest you are re-referred back to your dermatologist.

Lichen planus on the vulva is usually a chronic problem and may require periodic treatment. Regular long term emollient is helpful. Dermovate usually works and can be used long term intermittently. The frequency of Dermovate use is variable, but typically it needs to be used once or twice per week and then daily every now and again for a week or two for a flare up. Sometimes it is possible to go months without using it. The condition may eventually resolve so it is worth lengthening the intervals from time to time to see if the dermovate is still needed. A persistent sore area, ulcer or lump which does not resolve with dermovate over 1 month needs checking by a doctor.
