My scrotum has recently become more swollen and painful, I am an alcoholic but this has not been a problem until the last couple of months

thanks for this - you really need to see your GP about this as there are a few possible causes for this. The most likely is that you have developed a hydrocele, which is a harmless collection of fluid around the testicles. But because you mentioned your alcohol issues, it is worth checking that this is not related - occasionally high alcohol use can cause fluid to form in the abdomen (ascites) and this can track down into the scrotum. I would usually want to examine you and probably arrange a scrotal / testicular ultrasound scan.
