narrowing down the cause?

let's see... i've got a number of urogynae things going on. I went to a heavy menstrual bleeding clinic, and turned out i have fibroids in a anteverted uterus, i had a three inch (gah, yes, inch, it was scary to see) that was holding my cervix open, i seem to get an infected um, is it called meatus? the visible end of the urethral opening (it becomes red and sore with stinging and then goes blanched and stinging) quite often (i have some colorectal issues which i imagine contribute to some of the infection style problems). i'm also taking venlafaxine and propranolol and nifedipine in the course of a day (i've been told all three can affect urinary symptoms).
So. I have what seems to be ... i don't know what it is, that's why i'm tapping here! i feel the need to wee, but sometimes the act of standing up causes what feels like a wee bubble to overflow, and then there is no holding the stream. this doesn't always happen, and i don't always have an infection visible when it happens. and then sometimes i have no indication of a need, just wee. i'm also up at least two, maybe up to four times in the night needing to wee, urgently. sometimes i am successful in the travels, and sometimes i don't make it. if i take an antihistimine, i can sleep through the end of the dose before waking up needing to go (about four hours), otherwise, it seems like a two hour cycle.
i want to try and figure out why.
but i don't know what are the variables i need to test for, and how safely i can juggle medicine whilst experimenting, and which structural things need to be looked at, maybe not by me in a mirror.
can you help?

I would say that the fibroids can press on the bladder causing overactivity & leakage. I would suggest you discuss this with your urogynaecologist or GP to see if she can take an anticholinergic. As I don’t know if you are retaining urine I can't advise further.
