I sometimes see patients with urgency symptoms after bladder outlet surgery (such as TVT) - my first line investigation is often urodynamics to see what the cause of the incontinence is and treatment can be directed from there.

You need to discuss this with your specialist to review all of the options and to make an objective decision. One option is to consider a sling using your own tissue, which is put in quite tight, but there is a high risk of difficulty voiding after this - but that may be a better option for you. If that was the case, you would have to then intermittent self-catheterisation.
