Post-ejaculation pain; unique problem

This issue developed around age 18-19 (29 now). It’s extremely predictable. Any time there is any liquid in my bladder, at all, and I ejaculate, I within minutes have pain in my urethra and an incredible urge to urinate. The pain and irritation is temporarily satisfied by urination but then quickly becomes much worse where I have extreme pain that is only temporarily relieved every time I urinate (provided I have something left) followed by intense pain. Lasts about 30-45 minutes then I’m okay. Redness around tip of penis. Happens every time no exception. If I make sure I have a clear bladder when I ejaculate (empty when I start, come down when I’m about to finish, empty again, then come back up and finish) then it does not happen. Obviously seriously affects my sex life, can’t really stop and start like that when I’m having sex and if I’ve been drinking alcohol or caffeine which fills my bladder quickly I’m pretty much helpless.

It’s been diagnosed as chronic prostatitis and then I was told it was decidedly not prostatitis and then chronic Prostatitis again and now decidedly not Prostatitis again. Current diagnosed as a pelvic floor disorder (current doctor does not like feedback or disagreement, is convinced that it can’t be Prostatitis because I never have pain with urination or issues with urination except after ejaculation). Have had two scopes up urethra into bladder, one scope to examine prostate, and colonoscopy. Nothing abnormal. Have undergone sporadic pelvic floor physical therapy. Felt like some mild relief of symptoms but hard to tell. Currently taking baclofen twice a day with same results.

What depresses me the most, other than it seriously affecting my life, is that every doctor I go to, even specialists with sexual dysfunctions, has never heard of such symptoms. I wish it was in my head but it is very very real. I can’t even find any post online where someone has identical symptoms. Sometimes some remotely similar issues (they always have chronic Prostatitis) but never the very specific, very consistent and predictable issue that I have.

Sorry to read about your symptoms and of the frustration you are experiencing not being able to have it resolved. Have you tried anti-inflammatory tablets taken before sexual activity?
