ENDOMETRIOSIS AFTER HYSTERECTOMY

I had quite aggressive endometriosis and fibroids which resulted in hysterectomy aged 25 yrs. They had grown all over my other organs, and womb was size of football when they took it out. 5 yrs later they were back, the surgeon had left me one ovary, and again , growths all over other organs and ovary. They even stuck my vagina and anal passage together. Had that last ovary removed in 2005 and cleared all other organs of growths. Now I am getting lower abdomen pain again feels just like endo pain. Even ended up in A&E as pain so acute. I came off HRT 3 yrs ago after being on it for 25 yrs. Surely it cannot be back again, I have no more gynae bits left? I am 53 now.

Endometriosis in post menopausal women is uncommon, and as a result unfortunately poorly understood. It is generally felt to be less active in the postmenopausal period, however, it does have the potential to reactive if given appropriate stimulation. Post menopausal endometriosis may be enhanced in the presence of higher circulating level of oestrogen for example with hormone replacement therapy (HRT). There has been some limited evidence that unopposed oestrogen therapy was found to reactivate symptoms.

In your case I think it would be important to arrange further investigations to determine the cause of pain. It is difficult to know without further information whether it is related to endometriosis. This may initially be with an abdominal and pelvic ultrasound, but may also require MRI imaging and blood test to assess bowel and bladder function. Discussion with other specialists such as gastroenterology or colorectal team may also be indicated depending on the results.

From what you have described it sounds like all residual endometriosis was removed, however, you have had several surgeries ( which is often the case in endometriosis due to the chronic nature of the disease) these could have contributed to scar formation which can also result in pain.

THANKS for your reply. I have had 20 gynae surgeries in total to deal with the problem, so agree there must be scar tissue. A&E suggested an ultrasound, that is how they found the regrowths on my ovary last time. Thanks for your advice.
