Newly diagnosed with Lichen Sclerosus

I am newly diagnosed Jan 2024 with Lichen Sclerous age 40, my gynaecologist advised to use ClobaDerm for two months then stop. I’m currently getting more split/sores appearing daily and tears at the fourcette. I am only using the hydromol ointment now. Do I require steroids as this is considered a flare and how long for? Moving forward should I be using the steroid regularly to prevent the LS further? Thanks Verity

Increase in fragile skin/splits/soreness can be a sign of LS being active. Initial treatment for three months- daily x 1 month, alternate days x 1 months then twice weekly x 1 month - is recommended from the British Association of Dermatologists https://www.bad.org.uk/pils/lichen-scle ... n-females/.

After this, doctors will often advise on an individualised basis after seeing what response the initial treatment has had. It is unclear whether using treatment twice weekly for maintenance will prevent the LS from flaring further. This is currently being investigated by the PEARLS trial https://www.nottingham.ac.uk/PEARLS/.

Please discuss with your doctor the best topical steroid routine for you based upon your signs and symptoms.
