Low anti-mullerian hormone count & fibroids

I really need help and support on two parallel issues!
Firstly, my fibroids has come back (had them removed in June 2014) and it's grown back to 4cm. It causes me excess bleeding and cramps. :cry:
My second issue is that my husband and I are trying for a baby but when I had a blood test, the doctor told me that I have a low anti mullerian hormone count of 2.3 (meant to be 10 as I am 37 years old)
Not sure, where to go with this information? I've been told that NHS won't treat my fertility needs...But I can't afford the private care either!
What shall I do? I really want to be a mother but not sure what to do! I would be extremely grateful is some can give me some advice on the above.
