Food Allergy Dissertation Questionnaire

Hi All,
Hope this is okay to post here.
I have a dairy allergy so for my dissertation at Nottingham Trent University I have decided to research how we could improve the restaurant experience for people with food allergies.
I am posting this here as I am trying to target people with food allergies to gauge their experiences of eating out. All answers will remain anonymous.
It it only short, will take about 5mins to complete and will be a great help!
