Sounds like he has chronic urinary retention leading to an atonic bladder.
Would recommend a holep procedure over a PVP as it removes more tissue and gives him the best chance of getting going again with the normal passage of urine. Both of these have less chance of bleeding than a Turp.
Despite whichever procedure he chooses he may still not be able to pass urine and be left with a permanent or intermittent catheter as the bladder may not have enough power.
