Not quite sure what you mean by vacuole, I'm sure something can be done
could be a hydrocele (fluid around the testicle) or epididymal cyst or a varicocele (varicose veins of the testis)
you need a scrotal ultrasound scan and a urine test.
Ask GP for a urology referral to see if anything can be done..
