Great question. I have been often asked to help with the treatment of lichen simplex - a patch or scattered patches of itchy skin that get scratched and rubbed so much that the skin thickens up with what is called "lichenification" (see below* for more on this). Treatment with emollients and topical steroids is usually recommended but these will not work so well if habitual scratching is happening - and easily happens! There is an easy to use treatment called "habit reversal" that deals with this, allowing the creams to clear up the rash. If you are prone to getting stressed, this can be a complication too, and if you learn about managing stress, the treatment of lichen simplex will prove more effective - that's what we have discovered in our clinic! Lichen simplex eczema can be very difficult to treat. I agree that the most important part is to stop scratching if it is at all possible. In addition regular moisturisers and as required steroid ointments, of an appropriate strength, can be useful. Sometimes I inject steroids or use phototherapy and even powerful medicines.
