Please advise

I have been unable to pass normal sized stools for 8 years, following a gastritis based bloody diareha. Colonoscopy and endoscopy was done then.
I have been straining a lot and everything always comes out oval or flat shaped. I have had anal fissures a few times a year since 2014 when over eating white breads. I got a fissure recently and was given a cream. I have stopped straining but I am now getting a lot of constipation and take stool softeners. My diet is very healthy though, good amount of fiber.
It feels like if the stool is big it wont come out. The first one to two inches of stool seems to be left over from the day before as it is often twisted, tough, and slow to exit - the rest of the stool is a bit smoother. After passing a moderate oval/ flat stool i will eventually get mushy stool pieces, like what might have been shaved off of the first stool.
Now here is the kicker: I was given a prostate exam and for two days afterwards i had fully formed round stools with no straining. Basically perfect bowel movements! Something i haven't experienced in years. But then it just went back to the same old problems.
The only thing i have found online is maybe I have a very tight anal sphincter and the prostate exam loosened it temporarily. I don't know. I told my doctor all this and he didn't seem to get it, just advised restorolax and then metamicial.when regular. Also sent for colonoscopy, but that wont be for a while. If anyone has any ideas please tell me. Thanks - i get constipated because i cant get normal diameter stool out and resort to softeners.
