There aren't any until it is advanced, which presents a major problem since it needs to be detected early in order to achieve a cure from treatment. This is why regular testing with PSA is so important.

it's the only way to directly examine the prostate - believe me, as a doctor, I would really prefer not to have to do this as well!

it won't be long though before this method of examination is banished to history - we use a rectal examination to check the size of the prostate when treating symptoms - but we know that this is a poor test and is pretty inaccurate - and to check for signs of prostate cancer - once we have a better test than PSA alone, we may be able to stop. These tests may be along soon - adding things like simple genetic markers alongside PSA may improve the performance of blood testing sufficiently that I can finally stop pulling on those gloves...!

A key point here regarding symptoms and prostate cancer is that cancer grows on the outer part of the prostate and usually causes no symptoms until late. Benign Enlargement of the prostate occurs in the inside part and gives symptoms earlier.
Thus men with urinary symptoms usually havent got cancer but are worried about it, and men with no symptoms are often falsely reassured.
