Early Menopause & Bone Density

On behalf of Mary:
I've gone through early menopause and other than night sweats didn't really suffer any other symptoms. I therefore haven't taken hrt and I'm now concerned that I might suffer with bone density problems in later life. Is this something I should be concerned about and is there anything else I could do to prevent this without having to take hrt?

Dear Mary

Thanks for the question
The main concern is Bone loss (osteoporosis)

I would recommend
Life style and exercises
Healthy diet
Calcium supplement
If early menopause and not had HRT I would recommend Bone Density scan (DEXA) to assess if bones are fine

There is still a benefit from HRT, I would strongly recommend it (If you have no contraindications like Breast cancer or blood clots)

I would recommend you see a gynaecologist for that

I'd agree with the advice given so far perhaps with the addition of bisphosphonates if you're particularly concerned about the risk of osteoporosis as they do offer additional bone protection. The evidence for vitamin D supplementation is less robust (although we're all probably a bit deficient living in the UK) but anecdotal reports suggest it can give a sense of well being particularly over the winter months. Away from osteoporosis you may wish to consider Vagifem pessaries (used twice a week) these are very good at relieving the symptoms of vaginal dryness, strengthening the pelvic floor muscles and reversing the menopausal effects on the bladder (which may take a number of years to manifest) It's COMPLETELY SAFE its not HRT and if used for a year equates to the same dose as HRT for 1 day! I recommend it for all my patients who've had a pelvic floor repair but there's no reason why it shouldn't be used prophylactically to prevent these problems developing.

Your age and how long ago your periods stopped is very important since it still may be worth considering HRT for bone health, and may be more appropriate than the use of bisphosphonates.
Having said that, healthy diet and exercise as well as not smoking are hugely important.
It is strongly believed that for most women under the age of 60, or within 10 years of the menopause, HRT provides more benefits than risks for bone and heart health, not just for control of menopausal symptoms.
