Cancer? for insurance purposes?

In 2011, my PSA was high, had biopsy with a borderline GLSN score of 6.0. I consider prostate removal but decided to watch and wait. Since then I've had PSA checked and seen my doc yearly. I also have a very enlarged prostate which doc feels impacts my PSA. He put me on Proscar which lowered my PSA.

How do I answer this question on an insurance form:

In the last three years have you been diagnosed with cancer or treated for cancer? Yes/no?

You'd better ask insurance specialist to be on the safe side.
