I suggest he goes to see you GP and requesting an urgent ultrasound of the scrotum , It could be due to infection, but will need to make sure not other causes like cancer . Get an appointment ASAP for him to see his GP to be examined and the an Ultrasound . Hope this helps

There are no hard and fast rules in the older man, where less frequent testing of PSA is advised
