Hi,
You have back pain and this couses becouse of stress on the muscles in the back of the neck and upper shoulders.
So, i suggest you some treatments
1) Relaxation : Trying to relax
2) Keep moving
3) Do Exercise regularly
4) If necessary do Surgery on back

A portion of fruits and dry fruits is a must on all your 7 day meal plan. Avoid flour and junk readymade food and fast food. Try cooking at home and you do not need to count your calories as everything is healthy even if it might be a little high in calories.This has helped me loose a lot of weight....

If you have back pain, stick to low impact exercises, such as Yoga, Tai Chi, back strengthening exercises, swimming, water therapy, etc.

Try to eat meals after immediately after exercising or a work out. You can also eat a bit of rice at night to put on some weight.

You can try training yourself for bowel movements at certain times during the day—such as after meals. This may take time but after a pattern is set, it will be easy. pelvic floor exercises and biofeedback or electric stimulation for bowel control. Hope this helps.
