What is included in your child's Individual Healthcare Plan?

What is included in your child's Individual Healthcare Plan?

My son has an Individual Healthcare plan at school for his eczema and also a seperate one for his food allergies and asthma.

I was just wondering if your child has one for eczema what things are included in it. I will share what is in my son's as it may be useful for other people.

It obviously has his name, DOB, which class he is in, home address, medical condition, emergency contact information for myself and also contact information of his GP, Immunologist (the specialist we see) and his eczema nurse.

His daily care requirements that are listed on his plan are:

To sit on a small chair in assembly so that he does not become as hot.
Remind James to take his jumper off when it is warm inside.
To use his fan in class when he feels warm.Encourage James to rub or pat instead of scratching.
James is not to sit near a radiator.
Staff to encourage James to drink plenty of water.
If James is very itchy a member of staff is to take James outside for 5 minutes to help him cool down which also helps calm the itch.
When James's legs are sore he is allowed to stay inside during playtime and lunchtime.
James's emollient is applied in school at 10.30am by myself and again at 1.15pm by a member of staff.

We also then have that if at any point James is extremely distressed, in pain, shows extreme reddening and discomfort then staff are to contact myself for further advice. Which usually means I will collect him. Whilst I am on my way though they will apply more cream and also administer 10ml of Loratidine.

We also have to be informed if he has had any contact with somebody that has a cold sore, chicken pox or measles due to the medication he is taking.

All of James's Individual Action Plans are kept in a binder in the office. This way it is easy to grab in an emergency situation.

Mine is much simpler :) instructions on how to shower and moisturize.. :)

We have a seperate form that was done by James's community nurse with instructions on how to apply emollient. It was signed after the nurse gave training to the Teaching Assistant who applies his cream at school. This is also kept in James's file.
